What is a Medical Weight Management Program?
Our Medical Weight Management program is a patient-centered and multi disciplinary method to weight loss and weight maintenance with the goal of prevention or treatment of diabetes type 2. We follow the most recent evidence-based guidelines provided by the American Academy of Bariatric Physicians and the Treatment of Obesity Society. Dr Constant will lead the program with a team of dedicated health professionals.
Our Medical Weight Management program is a yearlong commitment to help you develop new habits to improve your physical and mental well-being. It is not a cosmetic weight loss program. Our main focus is to help you reduce your risks of diabetes, hypertension, and heart disease. If you are already diagnosed with one or more of these conditions, the program will help you better manage your chronic diseases. Our goal is that you acquire and maintain long-lasting habits to improve your quality of life.

Who Can Participate?
This program is open to both men and women over the age of 18, at risk for or with diabetes. It is also open to post-bariatric surgery patients who desire weight maintenance. Patients with eating disorders are not eligible to participate.
How Does the Program Work?
The program starts with a private consultation with Dr. Marie Andrine Constant, a primary care physician and a public health practitioner. She will review your medical history, assess your readiness to change, and help you define your goals. Depending on the complexity of your medical problems, she may request a clearance from your primary care provider or she may refer you to a bariatric surgeon.
It is a year-long commitment with biweekly or monthly visits depending on your needs. At your first visit, you will get a medical check up, with an EKG and labwork for cholesterol and diabetes screening. We will design a customized plan that takes into account your eating habits and your daily routine. No two people will have the same plan. You will learn about the effects of foods on your mood and your cravings. In the end you will know yourself better and eventually choose a path to improve your life at all levels.
Do You Prescribe Medications?
We do prescribe medications under specific conditions. It will depend on your medical history and your needs. Please note that your subscription fee does not cover medications. You will be given a prescription to bring to the pharmacy of your choice. If you are prescribed a medication to help you lose weight, you will be required to come to the office every month for a medical check up.
How Much Does it Cost?
This is a cash-based only program. The first three months are billed at $75/month and are nonreimbursable . After that we help you maintain your program at a reduced rate of $60/month by automatic withdrawal via a credit card or a bank account.
We will inform your primary care provider of your progress unless you ask us not to. We encourage you to discuss your lifestyle changes with your provider as you may require an adjustment of your medications.
